Researchers should ensure that all applicable forms, research documents, and supporting materials are complete and consistent before submission.
Ethics Review Application
Complete the appropriate REC application form.
Use the latest version available from the Forms & Downloads page.
Submit the complete research proposal or approved protocol.
Participant Documents
Include documents related to participant recruitment, information, and consent.
Provide the consent form that will be used with research participants, when applicable.
Include relevant information that explains the study to prospective participants.
Include recruitment letters, invitations, advertisements, or related materials, if applicable.
Research Instruments
Submit the tools that will be used to collect research data.
Submit the final or proposed survey questionnaire, if applicable.
Include interview questions, focus group guides, or similar instruments.
Include any forms, checklists, scales, or tools used in the study.
Supporting Documents
Additional institutional or researcher documentation may be required.
Provide the appropriate endorsement when required by institutional procedure.
Submit researcher qualifications or CV when required by the committee.
Additional permits, approvals, permissions, or supporting materials may be requested depending on the study.
