Participant Information Sheet

Include relevant information that explains the study to prospective participants.

Recruitment Materials

Include recruitment letters, invitations, advertisements, or related materials, if applicable.

Submit the tools that will be used to collect research data.

Survey Questionnaire

Submit the final or proposed survey questionnaire, if applicable.
